Survey Requirements Planning Checklist
- Type of survey: Employee, customer, market research, 360-feedback, training needs assessment, other.
- Design assistance required (none, help us refine questions, help us design the entire survey and related communications)
- Approximate number of items in survey (each item in a table count as one item)
- Type of items:
- Level I: Choose one, choose one grids (tables), choose all that apply, check-box, text field, validation, multi-page surveys, branching/skip patterns
- Level II: Ranking, forced ranking, rotations, computations, piping, and other special functions.
- Level of security required and type (no password, single password for all respondents, individual passwords)
- Number of different scales and demographics used (approximate)
- Approximate number of respondents. For 360-degree feedback, number of managers/employees to be rated, and number of respondents per person rated.
- Email administration required? If yes, number of addressees.
- Level I: Send out invitations and reminders; manage returned email invitations
- Level II: Pre-load information about addressees to personalize invitations and attach demographics to results (reduces number of demographic questions required within the survey)
- Type of reports: Live online results or offline analysis? Detailed reports or raw data set only?
- Level I: Table of response distribution for all items plus list of comments if applicable; basic cross-tabulations; raw data file in Excel or text format.
- Level II: Charts showing visual representation of data in Level I reports.
- Level III: Statistical analysis of results to identify distinct segments and drivers of survey outcomes; quadrant analysis to display importance/satisfaction relationships graphically.
- Level IV: Executive report with interpretation and recommendations.
